Player evaluations for High School Boys are Saturday, December 14, 8:30 pm to 9:30 pm at The King's Academy Gym.

The King’s Academy Gym is at 471 Arnold Mill Rd, Woodstock, GA 30188.

Notes:

·         Players will meet the coaches on the gym floor and run through a few things - please wear tennis/basketball shoes and bring a soccer ball and water and be prepared to move around.  (Please be sure to sign in at the front of the gym where concessions are usually sold)

·         Parents will meet on the stage at the back of the gym.  We'll talk about the season, fill out/collect paperwork, sign up for volunteer positions, look at uniforms and take payments.

REQUIRED ITEMS TO BRING TO EVALUATIONS:

·         Payment:

o    $350 for returning players with a uniform or $175 if you are paying in two installments (second installment of $175 due by Jan 27th).

o    $400 for players without uniforms or $225 if you are paying in two installments (second installment of $175 due by Jan 27th).

·         Copy of your child's birth certificate (unless you are a returning player and we already have it on file).

·         Copy of your child’s insurance card (both sides please)

·         Participant Code of Conduct Form - filled out and signed

·         Emergency Contact Form - filled out and signed

·         Affidavit of Eligibility - filled out and signed

·         Parent Code of Conduct – filled out and signed
